Thank you for your desire to participate on a Short Term Missions Trip. The process of acceptance on a trip is as follows:
-Complete this application
-Submit, with your completed application, a non refundable $100 deposit for your international trip, or a $50 deposit for a domestic trip
-The Short-Term Missions Ministry Staff and Team Leader(s) will review the application and references
-If necessary, applicants will be interviewed by the Team Leader and/or the Missions Ministry Staff. You will be notified by the Missions Office of the status of your application. Upon approval, you will be contacted by the Team Leader.

Crossroads Church Mission Trip Financial Agreement

1. Each trip participant will be responsible for paying or raising 100% of the costs of the trip. The money will be due according to the following timeline:
-A $100 non-refundable deposit is due with the application for an international trip. A $50 non-refundable deposit is due with the application for a domestic trip.
-75% of the trip money is due prior to the purchase of airline tickets. This is usually about 3 months before the trip date. An airline ticket will not be purchased for anyone who does not have 75% of the money in.
-The remaining 25% may be paid after the purchase of the airline ticket. If you have not raised all of the money for your trip 2 weeks prior to the trip, you will be required to write a check for the balance of your trip costs at that time. We will hold this check for 60 days after your trip to allow you more time to raise or pay your balance. After 60 days the check will be cashed.
2. A team member may be reimbursed for approved out of pocket expenses as they raise additional support. These costs will include but not necessarily be limited to airline tickets, shots, and visas. Receipts for reimbursement must be turned in within one week of your return from a trip.
3. Anyone who owes a balance on a previous year's trip will not be approved for a trip until that balance is paid.

WHAT WE BELIEVE ABOUT THE BIBLE:
The Bible is God's Word to all men written by human authors under the supernatural guidance of the Holy Spirit. It is the supreme source of truth for Christian beliefs and living. WHAT WE BELIEVE ABOUT GOD:
God is the Creator and Ruler of the universe. He has eternally existed in three personalities - the Father, the Son, and the Holy Spirit. These three are co-equal and are one God. (Genesis 1:1, Genesis 1:26-27, Genesis 3:22, Psalm 90:2, Matthew 28:19, 1 Peter 1:2, 2 Corinthians 13:14)
WHAT WE BELIEVE ABOUT JESUS CHRIST:
Jesus Christ is the Son of God and is co-equal with the Father. Jesus lived a sinless, human life and offered Himself as the perfect sacrifice for the sins of all men by dying on a cross. He arose from the dead after three days to demonstrate His power over sin and death. He ascended to heaven's glory and will return again to earth to reign as King of Kings and Lord of Lords. WHAT WE BELIEVE ABOUT THE HOLY SPIRIT:
The Holy Spirit is equal with the Father and the Son of God. He is present in the world to make men aware of their need for Jesus Christ. He also lives in every Christian from the moment of salvation. He provides the Christian with power for living, understanding of spiritual truth, and guidance in doing what is right. The Christian seeks to live under His control daily. (2 Corinthians 3:17, John 16:7-13, John 14:16-17, Acts 1:8, 1 Corinthians 2:12, 1 Corinthians 3:16, Ephesians 1:13, Galatians 5:25, Ephesians 5:18)
WHAT WE BELIEVE ABOUT MAN:
Man is made in the spiritual image of God to be like Him in character. He is the supreme object of God's creation. Although man has tremendous potential for good, he is marred by an attitude of disobedience toward God called "sin." This attitude separates man from God. (Genesis 1:17, Psalm 8:3-6, Isaiah 53:6a, Romans 3:23, Isaiah 59:1-2)
WHAT WE BELIEVE ABOUT SALVATION:
Salvation is a gift from God to man. Man can never make up for his sin by self-improvement or good works. Only by trusting in Jesus Christ as God's offer for forgiveness can man be saved from sin's penalty. Eternal life begins the moment one receives Jesus Christ into his life by faith. (Romans 6:23, Ephesians 2:8-9, John 14:6, John 1:12, Titus 3:5, Galatians 3:26, Romans 5:1)
WHAT WE BELIEVE ABOUT ETERNITY:
Man was created to exist forever. He will either exist eternally separated from God by sin, or in union with God through forgiveness and salvation. To be eternally separated from God is hell. To be eternally in union with Him is eternal life. Heaven and hell are places of eternal existence. (John 3:16, 1 John 2:25, 1 John 5:11-13, Romans 6:23, Revelation 20:15)
Crossroads Church is a member of the Southern Baptist Denomination, and we participate with fellow Southern Baptist Churches within the Cooperative Program. Crossroads Church is a local autonomous body of believers that is organized and governed by the model of the early church described in the New Testament. Crossroads is committed to being a New Testament Church that will reach a contemporary culture.
Commitment

If selected to be a part of this team, I make a commitment to:
a) Attend at least 75% of the training sessions prior to departure and after I return from the trip
b) Conduct myself in a manner worthy of the Lord while serving Him on the project
c) Submit to the team leader's and the host-on-the-field's authority
d) Refrain from any behavior which may compromise my witness (i.e.: abusive language, drug use, drinking alcohol, smoking, pursuing relationships of a romantic nature, etc.)
