I hereby give my consent for my child to participate in bigstuf Summer Camp 2018 of Crossroads Church, Newnan, Georgia. I understand that insurance coverage for accidental injury will not be provided by Crossroads Church and I agree that I will be responsible for any medical expenses that might be incurred because of accident or illness.
I hereby release Crossroads Church, Newnan, Georgia, its agents, employees, or volunteer workers from any liability for accidental injury or sickness which may occur to my child while participating in the above activity, including transportation to and from. I also give my consent to the sponsors to authorize emergency medical treatment for the above participant while trying to contact me at one of the phone numbers listed.
If the above person is required to leave in advance of the time when any furnished transportation leaves, either for medical reasons or for discipline reasons, then I agree to arrange and pay for the transportation back.

I authorize Crossroads staff/ volunteers/ workers to administer First Aid including over the counter medications as required for illness or injury. I hereby agree to allow him/her to participate in all activities that occur at camp. I realize that unanticipated and unexpected events may arise during and associated with camp activities. I voluntarily agree to to accept any and all risk. In the event that I cannot be reached in an emergency, I give permission to Crossroads staff/ volunteer/ workers to seek proper medical services and treatment.

I, as a parent, give permission for my child to spend one on one time with Fusion leaders on June 4-8th, 2018. This time includes riding to or from a church event. I understand that this is an exception to the general rule of Crossroads Church.
